Insurance
Thank you for choosing Maxa Internal Medicine Associates as your health care provider. We are committed to giving you quality medical care. Maxa Internal Medicine will continue to participate in all current insurance plans. If you are an existing patient of Maxa Internal Medicine you do not have to notify your insurance company. New insurance ID cards will be issued to you from your insurance provider.
For your convenience, we participate in many managed care programs. Check with us to make sure we participate with your plan. Because plans vary greatly in levels of benefits and hospital participation, please familiarize yourself with your insurance plan and its restrictions. (We make every effort to guide you in the right direction, but ultimately it is your responsibility to stay within the guidelines of your plan.) Please bring your insurance card with you. Be prepared to make full payment or at least your co-payment at time of service.
For Medicare patients, some of the commercial payors have Medicare plans that we may not be contracted with. Please contact your Commercial Medicare Plan to make sure that we are a provider before you choose a Medicare plan.
For marketplace insurance plans, please note that we are not contracted with several of these plans; therefore, you will need to call your carrier to determine if we are a listed provider.
